REGISTRATION FOR STUDENTS
NEW TO THE DISTRICT
Please read the below
items carefully. Certain requirements and paperwork must be done in advance so
that the registration process can be efficient and timely. Please do not
hesitate to ask questions. If you would like a counselor to call you, leave
your number and a message. A counselor will return your call as soon as
possible.
- RELEASE OF RECORDS
Complete a release form NOW and give it to the
secretary in the Main Office. The release form will allow us to send for
records from your former school immediately. Records are necessary to make the
schedule for next year.

STUDENT TRANSFER FORM (ILLINOIS
PUBLIC SCHOOLS)
If
you are transferring from an Illinois public school, request that the former
school sends us a completed form called the “Student Transfer Form.” This form
is required by law for enrollment. IT IS YOUR RESPONSIBILITY TO CONTACT THE
FORMER SCHOOL FOR THIS FORM.
- CUSTODY PAPERS (only if student does not reside with natural
parents)
Copy of divorce settlement describing custody arrangements
will suffice.
- PROOF OF RESIDENCY
In
order to register the student must be a resident of Community Unit School
District No 100 and live with a parent or legal guardian. Please arrange for a
conference with our principal if there is question concerning this.
- HEALTH RECORDS
Immunization and
health records for new students must meet state law.
- COPY OF BIRTH CERTIFICATE
Counselors will be available
August 5th, 6th, and
7th specifically to register
new students to the district. A guardian and the student must be present for
the conference.
